You need a Devil's Heart from raid bosses. Different hearts = different hybrid types:

Location: Medical Wing in HQ building, Floor 2

Look for the NPC named "Dr. Surgeon" near the operating table

Can I reverse Hybrid Surgery?

No. Hybrid Surgery is 100% permanent. You cannot go back to Human or Fiend once you become a Hybrid.

Do I lose my contracts as Hybrid?
